Signature Advantage (HMO SNP) , a Medicare Advantage Special Needs Plan, is pleased to announce it is becoming Abilis Health Plan . This brand change reflects a refreshed identity and continued commitment to providing high-quality, coordinated healthcare to members.
The only changes members and partners will see are the plan’s name, logo, and brand identity. All other plan details, including the contract number (H2400), benefits, premiums, provider network, and customer service contacts, remain the same. Members will continue to enjoy the same trusted care, benefits, and provider relationships under the new Abilis Health Plan name.
