Olmesartan alone or in combination with hydrochlorothiazide is a small molecule treatment being developed by Pfizer for essential hypertension. Olmesartan is an angiotensin II receptor antagonist, and hydrochlorothiazide is a thiazide diuretic. The combination is intended for patients whose blood pressure is not adequately controlled by either agent alone.
Olmesartan targets the angiotensin II receptor type 1, also known as AGTR1, where it acts as an antagonist. By blocking AGTR1, olmesartan reduces the vasoconstrictor and aldosterone-secreting effects of angiotensin II. Hydrochlorothiazide works through a different mechanism, increasing sodium and water excretion to lower blood pressure.
