Study Endpoints

Primary Endpoints

Incidence of ambulatory systolic blood pressure (BP) > 140 or diastolic > 90 (measurement on 2 separate days)
At baseline and up to 12 months

The change will be estimated and tested using paired tests (Wilcoxon signed rank test or McNemar test). The difference at each interval for the two groups will be tested using Fisher's exact test for binary endpoints or Wilcoxon rank-sum test for continuous. Data transformation to fit statistical assumptions will be done as needed. Multivariable models that control for potential confounders will be implemented using general linear and logistic regression.

Need for new or escalated anti-hypertensive medication
At baseline and up to 12 months

The change will be estimated and tested using paired tests (Wilcoxon signed rank test or McNemar test). The difference at each interval for the two groups will be tested using Fisher's exact test for binary endpoints or Wilcoxon rank-sum test for continuous. Data transformation to fit statistical assumptions will be done as needed. Multivariable models that control for potential confounders will be implemented using general linear and logistic regression.

Incidence of moderate-to-severe atherosclerosis of major coronary vessels
From month 0 to month 12

Defined as \> 50% luminal stenosis per the Society of Cardiac Computed Tomography. Change will be tested using paired tests (Wilcoxon signed rank test or McNemar test). Luminal stenosis will be measured on a per-vessel basis. Proportion of patients achieving \> 50% luminal stenosis of a major coronary vessel between each arm will be compared using Fisher's exact test. The percent change of maximal stenosis between the two arms will be tested using Wilcoxon signed rank test.

This study aims to determine if immediate postoperative medical suppression with Regugolix combination therapy (Rel-CT) following excisional surgery for endometriosis is superior to surgery alone.
6 months

The study will assess quality of life via change in total Endometriosis Health Profile 30 (EHP-30) scores following excisional surgery for endometriosis from baseline to 6 months postoperatively.

18-month Rate of Testosterone Recovery
42 months (18 months after completion of 2 years of treatment)

Rate of testosterone recovery (TR) defined as the proportion of participants achieving testosterone ≥ lower limit of normal by 18 months after completion of study treatment. Participants who withdrew from trial therapy (due to early progression, toxicity, patient/physician decision or death) before completing 2 years of treatment will be excluded from the analysis. Participants who require resumption of androgen deprivation, withdraw, are lost to follow-up, or die before 18 months of post-treatment follow up has been completed without having achieved testosterone recovery ≥ lower limit of normal beforehand will be included in the analysis and will be considered to be non-recovered.

Grade 3-5 Treatment-related Toxicity Rate
24 months. AE collected on day 1 of each cycle (up to 26 cycles in total, each cycle is 28 days)

All grade 3-5 adverse events (AE) with treatment attribution of possibly, probably or definitely related to study treatment based on Common Terminology Criteria for Adverse Events (CTCAE) version 5 are counted. Rate is the proportion of treated participants experiencing at least one of these adverse events as defined during the time of observation.

Proportion of participants that attain PSA nadir
up to 2 years post treatment

The proportion of men who undergo SUGAR that attain PSA nadir of \<= 0.2ng/mL compared to SBRT alone.

9-month Quality of Life (QOL) Score
At 9 months

The QOL assessed using Functional Assessment of Cancer Therapy - Prostate (FACT-P) questionnaire, defined protocol section 11.1.1: The FACT-P (version 4) contains 39 Likert-type items distributed over 5 subscales: physical (7 items), social/family (7 items), emotional (6 items), and functional (7 items) well-being, and the additional concerns related to prostate cancer scale (12 items). The FACT-P total score is calculated by summing all these 5 subscales and ranges from 0 to 156.

Proportion of patient experiencing dose limiting toxicity (DLT) and tolerable dose of relugolix and enzalutamide combination
2-3 years

Proportion of patients will undergo evaluation for dose-limiting toxicity (DLT) to establish a tolerable dosage regimen for the combined use of relugolix and enzalutamide in individuals diagnosed with high-risk locally advanced prostate cancer. The assessment hinges on identifying DLTs associated with this treatment using 3-12 patients from the safety lead-in cohort and continuously monitoring treatment-related adverse events throughout the DLT observation period, which spans 24 months, encompassing neoadjuvant and adjuvant treatment phases.

Proportion of patients experiencing Pathologic Complete Response (pCR) and Minimal Residual Disease (MRD) in Neoadjuvant ADT with the combination of Relugolix and Enzalutamide
4-6 years

Proportion of patients achieving a pathologic response rate, defined as the combined Pathologic Complete Response (pCR) Rate and Minimal Residual Disease (MRD) rate (MRD, tumor ≤5 mm), serves as the primary endpoint for the dose expansion cohort. This assessment will be conducted using Simon's optimal 2-stage design, enabling a determination of whether there are an adequate number of patients with objective responses to proceed to the second stage of the study (at the interim analysis) or to consider the drug for further investigation in a Phase III study (at the study's conclusion).

Frequently asked questions about Relugolix

What is relugolix?

Relugolix is a small molecule gonadotropin-releasing hormone receptor antagonist developed by Pfizer, Inc. (ticker PFE). It is being studied across prostate cancer and endometriosis indications, including androgen deprivation therapy, recurrent prostate carcinoma, and advanced or hormone sensitive prostate cancer. It is an investigational agent in clinical development.

What does relugolix target?

Relugolix targets GNRHR, the gonadotropin-releasing hormone receptor, and acts as an antagonist. By blocking this receptor, relugolix suppresses the hormonal signaling pathway that drives testosterone production, which is the basis for its study in androgen deprivation therapy and prostate cancer.

Who makes relugolix?

Pfizer, Inc., which trades under the ticker PFE, is the developer of relugolix. The company is running the clinical program evaluating the drug in prostate cancer and endometriosis settings.

What phase is relugolix in?

Relugolix is in clinical development and is investigational. Its trials span Phase 1, Phase 2, and Phase 3, including a Phase 1 study of relugolix plus enzalutamide in high-risk prostate cancer and Phase 3 studies in prostate cancer and endometriosis.

What clinical trials is relugolix in?

Relugolix is being studied in NCT06650579, a Phase 3 trial of relugolix plus abiraterone acetate versus leuprolide plus abiraterone acetate; NCT06463457, a Phase 2 trial with darolutamide; NCT06439524, a Phase 3 endometriosis trial; and NCT06130995, a Phase 1 trial with enzalutamide.

Is relugolix the same as Orgovyx?

Relugolix is also known by the name Orgovyx, including the listing Relugolix 120 MG [Orgovyx]. Other associated names include relugolix and enzalutamide, 40 mg relugolix, and combinations with 1 mg estradiol and 0.5 mg norethindrone acetate.
